What cold medications are safe to take with Warfarin? 

The following cold medications are considered safe to use with Warfarin, an oral anticoagulant. These include:

  • Guaifenesin 
  • Acetaminophen 
  • Dextromethorphan 
  • Nasal decongestants (Pseudoephedrine and Phenylephrine) 

Guaifenesin 

Guaifenesin is considered safe to take with Warfarin because of its good safety profile. This drug is an expectorant and it helps loosen up the thick mucus blocking your airways. 

Warfarin does have a lot of interactions with most medications, but Guaifenesin is considered safe to use. 

Acetaminophen 

Acetaminophen is the painkiller of choice to take with Warfarin, which reacts with the majority of painkillers. Acetaminophen can help manage pain and fever associated with the common cold and flu and you can take this medication 2-3 times a day while you are on Warfarin. 

Dextromethorphan 

Dextromethorphan is a cough suppressant which can be used safely with Warfarin. You can take medications like DayQuil, which is an OTC med and you can safely take it with Warfarin. 

It contains Dextromethorphan, Acetaminophen, and Phenylephrine (discussed shortly). The med can help counteract a variety of cold and flu symptoms without any interaction with Warfarin. 

Nasal decongestants 

Nasal decongestants can also be safely taken with Warfarin. The most commonly used nasal decongestants are Pseudoephedrine and Phenylephrine. There are no known interactions between these meds and Warfarin. 

What cold medications are not safe to take with Warfarin? 

Non-steroidal anti-inflammatory drugs (NSAIDs), commonly used painkillers, are not safe to take with Warfarin. This is because NSAIDs have their own blood thinning and anticoagulant effects (1). 

In fact, Aspirin is used as the blood thinner of choice for the management of symptoms after a heart attack. When these meds are combined with Warfarin, the consequences could be life-threatening as the bleeding time is significantly prolonged. 

This is why it’s recommended to never pair Warfarin with NSAIDs. Common NSAIDs include:

  • Aspirin 
  • Naproxen 
  • Diclofenac 
  • Ibuprofen 

Instead of using these meds for your pain, use Acetaminophen. If Acetaminophen fails to manage pain, talk to your healthcare provider for a safe alternative that you can use along with Warfarin. 

Some painkillers can be used with this anticoagulant, but dose adjustment and close monitoring are necessary.
