Does Wellbutrin interact with NyQuil? (3+ facts)

Does Wellbutrin interact with NyQuil? 

Yes, there is a moderate interaction between Wellbutrin and NyQuil. Wellbutrin can increase the level of Dextromethorphan, which is an active ingredient in NyQuil. The active ingredients present in the Original NyQuil formulation are

  • Acetaminophen- acts as a painkiller and also reduces fever
  • Dextromethorphan hydrobromide- acts as a cough suppressant
  • Doxylamine succinate- An antihistamine used for allergic reactions

Other formulations of NyQuil which interact with Wellbutrin are;

  • Vicks NyQuil Severe Cold and Flu: This formulation includes Phenylephrine, which is used as a decongestant. Combining Phenylephrine and Wellbutrin has an additive effect and can increase your blood pressure. 
  • Vicks NyQuil Cold and Flu Nighttime Relief: This formulation contains Chlorpheniramine. Wellbutrin increases the level of Chlorpheniramine and can increase the risk of side effects caused by Chlorpheniramine. 

NyQuil is not recommended to be taken routinely with Wellbutrin due to potential adverse effects. A lower dose of NyQuil is suggested when taken with Wellbutrin. It’s important to use NyQuil only as needed and not regularly.

An increase in Dextromethorphan and Chlorpheniramine levels may cause unwanted symptoms such as drowsiness, dizziness, nausea, vomiting, nystagmus, dry mouth, loss of appetite, vision problems, and difficulty urinating. What does the research suggest? 

Wellbutrin (Bupropion) is an inhibitor of the enzyme CYP450 2D6. This enzyme metabolises Dextromethorphan and Chlorpheniramine. Inhibition of the CYP450 2D6 enzyme causes decreased drug clearance and increases the levels of Dextromethorphan and Chlorpheniramine. (1)

One of the many studies has shown the effectiveness of combining Dextromethorphan and Bupropion in adults with major depression. (3) Two case reports have shown liver injury associated with using NyQuil for a longer duration, which contains acetaminophen. (4) 

Dextromethorphan contributes to the release of serotonin. Bupropion, being a CYP450 2D6 enzyme inhibitor, can increase Dextromethorphan levels, leading to serotonin syndrome. (5) NyQuil also has an effect on heart rate, which is dose-dependent; an increased concentration of NyQuil can elevate the risk of tachycardia. (6)

What are the side effects of taking Wellbutrin with NyQuil?

Combining Wellbutrin and NyQuil can result in mild to severe side effects. The severity of the symptoms varies from person to person, and it is recommended to avoid this combination if you are experiencing any side effects. Here are some side effects you may experience due to Wellbutrin’s interaction with the following;

Dextromethorphan: 

  • Dextromethorphan overdose or excessive intake can result in various symptoms such as breathing difficulties, blurry vision, bluish discolouration of nails, constipation, seizures, drowsiness, dizziness, stomach pain, nausea, and vomiting.
  • However, the common symptoms experienced by the majority of individuals with increased levels of Dextromethorphan are drowsiness, dizziness, nausea, and vomiting.

Chlorpheniramine: 

  • Side effects of Chlorpheniramine include drowsiness, dry mouth, nausea, vomiting, loss of appetite, constipation, headache, vision problems and difficulty urinating. 
  • If you have any of the above-occurring effects, stop taking NyQuil and consult your physician for management.

Phenylephrine: 

  • Combining Wellbutrin with Phenylephrine can cause an increase in blood pressure, so caution is needed when taking Vicks NyQuil Severe Cold and Flu if you have hypertension.
  • Be careful not to exceed the recommended dosage of NyQuil, as doing so may increase the risk of hypertension.
  • It is recommended that you consult with your physician regarding the appropriate dosage of NyQuil.

How to ensure safe and effective use of Wellbutrin with NyQuil? 

  • Taking a dose of NyQuil: Taking a high dose of NyQuil to treat your symptoms may increase your heart rate. It is recommended to use only a small amount of NyQuil and to approach your healthcare provider if your symptoms do not subside. Avoid taking more doses of NyQuil without consulting your healthcare provider.
