Can you crush Azithromycin tablets?

You can crush Azithromycin tablets as long as they are immediate-release tablets. Tablets which are sustained-release or delayed-release should not be crushed or else you’ll ruin the entire concept of the modified-release dosage form.

It can also cause some side effects and could possibly put your health at risk. This is why it is not recommended to split or crush extended-release dosage forms (1). 

Precautions to take when crushing immediate-release Azithromycin tablets

Crushing immediate-release Azithromycin tablets also comes with a few precautions. Let’s take a look at them:

Do not crush the tablet and save it for later use

It is not recommended to crush your Azithromycin tablet, mix it in a liquid, and save it for later use. You’re supposed to take your dose right after you crush it. Leaving it exposed to the environment can result in drug degradation. 

Do not add it to dairy products or products containing magnesium 

Although Azithromycin can be taken with dairy products, it should not be added to a liquid which contains calcium and magnesium. This is because these elements can affect the absorption of the antibiotic.

The effect may not be similar to Tetracyclines, but it can affect Azithromycin absorption to some extent.  

Do not mix the crushed powder in a huge portion of a liquid

It is recommended to add the crushed powder in a small amount of liquid that you can consume easily. Azithromycin is extremely bitter in taste and the tablet compaction usually masks that taste and the bitter drug is released in your stomach. 

When you crush the tablet, the bitter drug is already out in the open. It also makes the liquid bitter that you’re adding your drug to. This is why it’s best to take a small portion. It is also important to not mix the crushed powder with any other medication.

If you are taking other meds with Amoxicillin, like Acetaminophen for the relief of pain and fever associated with your infection, make sure you don’t crush and combine the two drugs together. They should be administered separately. 

Final words

To sum up, you can crush Azithromycin tablets if absolutely necessary. However, the best way to take medications is to swallow them whole with a  glass of water. 

Some people do have phobias when it comes to swallowing tablets and it is understandable. For such people, proper tablet crushing should be practised and they should ask their healthcare professionals before they start doing so.
